Boarding Checklist
- Agreement/Sign-up form (both owners initials liability
fields)
- Vaccines Records : Distemper/Parvo (DHPP) , Rabies,
Bordetella
- Leash & collar with ID tag
- Dry food with food bowl
- Bed/cot/rug (if you don't have, it's ok)
- Cash Payment
Final notes:
- - Proof of rabies, DHPP, bordetella vaccines are to be scanned and emailed no later than 2-days prior to arrival
- - If fleas/ticks are found on your pet upon arrival, DBar Dogs is authorized to provide flea/tick medication immediately and owner(s) will be charged $35 for each pet (maximum $35 per pet per stay).
